The better part of my childhood, calling something gay meant it sucked, and calling someone a fag or homo was a really absurdly common dis. If someone called you a ######, that was an escalation no doubt.
Maybe IIRC correctly, my senior year the general student body stopped using homophobic terms as a general rule, but without doubt it did remain a common insult.

Not just kids.
It is really illuminating when you watch 1980s or '90s sit-coms just how prevalent and socially encouraged homopgobic slurs were. It is a weird experience of remembering how much I loved this show, to feeling embarrassed about how much of what passed for "funny" was at the expense of the gay community.

This happens a lot to people who just aren't comedically sharp/spontaneous. You see it a lot working in IT services because you have this mix of people who are natural talkers mingling with people who generally ... aren't.
Someone in the latter category starts going down the path of what they think is going to be witty banter; this clever riff that'll provoke a sensible chuckle from the other participants, but they quickly realize they're speaking faster than their train of thought is prepared to go. They realize they need to finish this stupid joke quickly and -- before they know it -- what started off as a mundane attempt at a casual joke has crossed four lanes of freeway traffic, missed the exit, and plowed into the ditch. You can see it in their body language.
Usually it just ends up as this kind of awkward remark that prompts someone to let out a weak but polite acknowledgement and someone else will very quickly take back the reins of the conversation. And sometimes it ends up having an unintentionally inappropriate conclusion, because that was the phrasing their mental game of word association came up with to get them out of this mess.
