I eat out a lot, much more often than anyone I know. I've eaten in some kind of restaurant or food service establishment 181 times so far in 2012 (and for some reason I've kept track). Of course sometimes this includes little things like donuts or that 3 AM junior chicken you can't go without, so let's call it 150 times.
I tip 15-20% across the board when I dine in, $0 for takeout orders and $0 for fast-food which I don't really eat that much of. The most important thing to me is the food. If the food is good and the service somewhat adequate, I'll go back often, and I'll continue tipping 15-20%.
If the service is bad and the food is good, I'll tip my regular amount and give it another shot. I'll never visit a place with good food less than three times before I decide not to go back because of shoddy service.
There is one place I visit quite frequently that has exceptionally abrupt and I guess some would consider poor service. I visited probably 8-10 times early last year, tipped 15%, and got relatively poor service each time. The food was so good though that I couldn't stop going, so I started ordering takeout. I now go in about once a week to pick up my takeout order and I don't tip a dime (I never tip on takeout). I receive a warmer welcome and even sometimes some casual conversation more now than I ever did when I dined in. No clue why, I haven't tipped one cent at the place in a year (~40 visits). I'm recognized and greeted warmly when I go in, and I don't sense any sort of resentment from the staff that I don't tip on takeout. Maybe they just appreciate that of all the places I can take my money in Calgary I choose to take it to them once a week.
If the service and food are bad, I swallow my pride, tip the 15% and just simply don't return. Lots of other choices out there.

I just don't get what she's trying to accomplish. Even if everyone here agreed with her, so what? It's like she's some kind of food preacher, convertin' the heathen who dine with their eyes closed to great injustice until shown The Way.

I worked as a cook in the industry for 8 years. Before I left Canada I took my parents to the Keg for dinner. My dad wouldn't stop about how good his steak was. Even though I was paying the bill, and tip very good for good service, I made him talk to the manager, go into the kitchen and personally tip the girl who cooked his steak $20.
As a cook, you take home s**t in tips. I was only once tipped personally as a cook, and it was an awesome experience. I will also note I was a sous chef, worked a grill cooking steaks in a upscale restaurant where peoples bills normally where $500-$1000 and even though my tips I took home where pretty good, I was never personally tipped for food I cooked at that establishment.
Service is great, tip for service, but if you are tipping because your meal was amazing, talk to a manager and tip the person who cooked that food so the money goes in their pocket, not the waiters pocket. That cook is only taking home about 2-4% of the tip you leave.

14. Make sure you are bringing enough of what the customer has asked for. My husband asked for ketchup and the waitress brought out an almost empty bottle, not enough for him to even get much out. Also, if someone asks for a "SIDE" of something, bring a "FULL" side, NOT "a HALF side." It's ridiculous that now I am having to tell my server, "Fill it all the way." I think that's just ridiculous. If I ask for a "side", I want a FULL SIDE. Especially if I asks for 2 sides of mayo, don't bring me 2 "half-sides" of mayo, because then it's like only having ONE side of mayo, so then you have to make another trip to get another side of mayo as I specifically ordered it. It's not like the server can't SEE that it's not full. Bring customers enough condiments so you WON'T have to make extra trips. Do you honestly care about the pennies it cost the restaurant to maybe give too much or do you care about your tips? I think a customer would rather have more than enough, than not enough, wouldn't you?
