I made my husband a "storybook". I took pictures of us and receipts and stuff and put it in a photo album and wrote out a fairy tale story to go with the pictures.
The next year he gave it back to me with the story continued with his side of the fairy tale and pictures we took during the following year.

I second this idea. My wife and son went to PEI for the first week of Feb and I re-decorated our son's room. She had tears of joy/suprise when she returned, he was more interested in the new toys.

I made one of my old GF's a travel guide of places we wanted to go together in the future.
I took pics of us that I had and photoshopped them into locations around the world such as infront of the Taj Mahal, Brazil, on an Alaskan cruise etc.
Then I added some captions about things we would do and see in these places together.
One location per page and then had it bound like a magazine.
She loved it, but we never ended going to most of the places, and broke up. haha
P.S> completely unrelated, but I'm watching a crack deal right outside my office door right now and it's very entertaining.

When my boyfriend got me Flames tickets for Christmas, he put the tickets in a balloon and inflated the balloon for wrapping paper. When he got me a necklace, he put the box in a box of chocolates with four chocolates removed to make space for the necklace box. Not really present ideas, but wrapping ideas I guess!

I love to cook and make stuff for her. She's trying as well, so I made her a homemade recipe book with all of my favourite creations one year.
For this year, I have combined Valentines day and her birthday with a whole bunch of little gifts that she'll recieve over a 2 week period.
Also, from the Banff food thread, I'm taking her to Banff this weekend for her birthday
