I find Europeans in general have fewer inhibitions when it comes to privacy and personal space. I was in a public bathroom in Europe before and the cleaning lady came in and started cleaning the urinal next to me while I was midstream. I don't know, maybe it was just a one-off weird thing.

nope, not a one off.
Thats just what they do and dont think twice about it.
Also, if you recall, they probably asked for some sort of donation for using the facilities.

Based on the figures we've been cited, "fixtures" and "toilets" were used interchangably, so we believe they include urinals.

But trying to build flex-bathrooms or unisex with toilets instead of banks and banks of urinals is stupid. I'm not even a big beer drinker at events (I always drive), but this to me just seems like common sense. It expedites the process. The best arenas and stadiums I've been to have mass-urinal access.

I think you're misunderstanding the flex bathrooms (or I am). My understanding is:
Taylor Swift concert? Throw a "Women" sign up on that particular bathroom.
Hockey game? Throw a "Men" sign on it. Yup, toilets only, but better than dead space of women's washrooms needed to have capacity for a subset of events.
