An E400 with a stage 1 would be awesome so my vote is do it!

I had my E550c (M278 engine) tuned to 550HP/705FT-LBS (Stage 2, stage 1 was ~503HP) and it was glorious. Reliability was unchanged, which is to say it was trouble-free; I did the regular maintenance and it was just fine.
If you're gonna do a tune on the E400, do a Stage 2 and try to get downpipes done at the same time. It'll dramatically improve the responsiveness and the end result.

Brother in law tuned his Forrester and later he had some engines problems. Can't say for sure it was because of the tune, but can't rule it out either. Mechanics never could figure out the issue either. He traded it in so someone else's problem now.

Yeah, sold now, but my CLS550 had this tune as well and was a rocket and totally reliable for a decade.
I actually tuned my F-150 3.5 Ecoboost last week. The tune just came out for the 2024s through Ford Performance (I've been checking weekly for this tune for over a year). It's not as aggressive as a non-Ford tune, but it maintains my factory warranty. Adds about 115 ft-lbs of torque and 55 horsepower or something. $1300 all in and installed at the dealership. Again, not the greatest deal and these motors can make way more power, but I want my warranty.
Results are great. I have 33" tires so it'll never be a sports car, but I've gotta be in the low 5 seconds for a 0-60 and it is fast and monstrously torquey. It's faster than most sports cars from the 90s and it's a big truck. I think I'm over 600 ft-lbs now.

A completely unmodified car can have engine problems, tuning on its own isn't going to have a big impact on the long-term reliability unless you do something stupid in the tuning itself. How someone drives said car is a different story (and most people tuning their cars do it to drive them hard). Maintenance discipline, too.
And Beyond.ca is a ghost town these days, sadly.

More power under the curve than a 2014 E63 AMG, too.
The downpipes will also give you nicer fuel economy when you drive it nicely, though at the time I wasn't able to find decatted downpipes (or even sport cats) for the C207 E-Class.

I'd recommend you do your research on the car on forums/reddit, and hopefully there is enough history you know what you're getting.
In my case, I have a BMW M5 touring with a stage 2 tune. The same motor with same internals is used in other BMW applications with 150+ more HP... It's simply detuned in the hybrid setup my car has, so there is limited risk to adding the 150HP from the tune. I know from the non-hybrid variants that my motor is good for another 300HP before issues crop up.
Obviously this is a case by case analysis, but research and forum user experience how I have decided on tuning (and what stage/boost level) to go with in all my previous tuned cars, and I've never had an issue. I would caution against being a first mover on a non-OEM tune, but once there's a good user history built up, you usually know what you're getting, and what failures/warning signs to watch out for.

I find this really depends on the specific engine. I've had several cars tuned and never had a problem. The most important part is usually taking care of "Stage 0" and identifying the aging/likely to fail parts for your platform and taking care of those before jumping in.
