I'm finding today tough with Leafs/Habs and Leafs/Sens. Those teams don't make many trades!
Well there was one relatively recent trade involving a former flames defenceman...

These guys come in and it’s always a nice polite one. You serve them because you dont want to cause a scene. And then they become a regular and after awhile they bring a friend. And that dude is cool too.
And THEY bring friends and they stop being cool and then you realize, oh, this is a Nazi bar now. And its too late because they’re entrenched and if you try to kick them out, they cause a PROBLEM. So you have to shut them down.

The player doesn't have to have gone from one team to the other, just played for both... So, a particular goalie who started his career in Montreal, then went to St. Louis, then was on the bench when the Flames won the Cup, and eventually was traded to Toronto along with another well-known former Blue/Flame/Leaf/Hab would be a decent choice for the Toronto-Montreal one.

How the bloody hell did 3 other people put Kim Issel for an Oiler than only played on one team? He had four bloody games in the NHL! That's up there with Martin House as a Flyers goalie, the kid played a total of what 4 minutes in the NHL? Blows my mind how people get these obscure players... I mean, they played for my hometown teams (Issel with the Cape Breton Oilers, and House with the Cape Breton Eagles)

You just need to be at a game or watch a game where they so happen to do something or it’s their first game. Something notable. For example, I was at Blair Betts first game when he scored his first goal which turned out to be the game winner. Legend in my memory because i find it so fun to cheer for obscure one off things like that. So I know all the teams he ended up playing for because it was always notable to me.
I am sort of disappointed that they don't make it a bit more difficult with having to make it 3 different components. Like a 30 goal scorer or a first round pick who played for two different teams or played in a particular decade.

My PB as well with uniqueness 26! My most common choice was Monahan under Flames/OHL.
I love getting to use embarrassing Oilers like Yakupov, Dubnyk, or in today's case Steve Smith.
