Really bothers me. A lot. There is such an incredible focus on materials selection for engineers, techs, maintenance etc in every aspect of machinery especially those directly affecting people. To me this represents multiple massive failures that surely multiple people were complicit and CHOSE not to say or do anything to rectify the problem. Another case of “somebody” else’s problem, not mine. Combined with “that’s how stuff is done around here”.

How the hell could they only be sorting by size on the shelf and not material type. Boggles the mind. I've had to fight managers who would also stock things like o-rings on the shelf with no regard to material (makes a massive difference in the chemical industry). To see this in the aviation industry though, how the hell.

"Fore Machine Company ceased operations and closed in November 2021."
I should find the name of the owner and see what new business magically started operation in December 2021.

Will there be penalties? No, probably not, because they are bankrupt of course.

Bulgarian split squats and Romanian deadlifts will fix that. I made that grunting noise for a good two to three years before I fixed it.

I think it was a bad materials handling system that led to a human error.

When someone grabs the part from the warehouse to make it you have the highest risk point as it’s never tested again. So this company appeared to have all identical looking rods of the same diameter of steel beside each other and a person grabbed the wrong one and fed it into the machine.

Segragate different materials in fab shops. To me this is a system problem less than a negligence or human one

It's negligence when you make parts for aircraft/heli's and you are required to have a system that ensures that traceability.

Crap like this makes my blood run cold. I am on a project that has so much crazy alloy steel stuff going on and a lot of the gasketing etc is exotic. Ask me how much I love seeing Chinese material documentation.

My legs are super strong (skiing) but my back and core definitely need some work. I tend to skip everything but leg day

My wife was all set to bake up some fresh scones, and the heating element of the oven blew apart while it was pre-heating.

RGMG that now do I not only not have fresh, hot scones, I have to go oven-shopping.

I would think the negligence would fall on the manufacturer ultimately. Suppliers make mistakes and it falls on the manufacturer to do due diligence when they receive parts. I haven't been involved in the manufacturing of aircrafts or helicopters but I would assume virtually all structural metals purchased from suppliers would require MTR's and NDT on any welded parts.

I read that as they sorted by size then material. This creates a higher potential for error then if you sort by material than size.

If it is an external heating element it can be replaced in under 5 minutes and they sell replacements at any hardware store. My stove is 10 years old and I have replaced mine 4 times so far.

If it is an internal element, enjoy your new stove.
