Some people want to simply point to bad luck but this seems like too large a sample size. Bingo has pointed out in his game takes that there are some things the Flames have simply not tried on the PP (like switching sides on the point) which does seem a little perplexing considering the PP has the potential of crashing their entire season.
I am sure the coaching staff has their reasons for what they are doing. Unfortunately that doesn't equate to it working.

Watch the game tonight. Giordano will attempt the bump back to Gaudreau on the left side of the Flames attack. Gaudreau will dish to Monahan after gaining the zone, at which point Nashville's defenseman will break up Monahan's bank pass back to Giordano. It happens every single game, yet they keep doing it. Usually results in a chance the other way during the man advantage.

I don't think he is as effective in a stationary position trying to view the ice. Monahan can get into the right spots but he also seems to be a primary mark for defensemen to keep on a leash and not get into a hole. Brodie also is a great passer, but I don't see the instincts of a PP quarterback there.
I don't think it is a coincidence that the PP has looked better recently with the addition of Tkachuk (cycle machine), and Giordano (much more experience than Brodie). The Right hand shot is an issue, and so is the lack of movement and variety of threats. And right now confidence is a huge issue.

I totally agree, but I would add that I think Versteeg's addition to the powerplay seems to relieve pressure on the other catalysts, Gaudreau and Giordano. I think most often Gaudreau is holding the puck too long when the Flames are set up on the power play.

Yeah I think at this point the PP is in the players head. They are passing up opportunities to get the puck to the net.

I can't figure it either. Although to be fair, Johnny's biggest skill is stickhandling, and he still does that very well on the PP (I have seen him retain possession a bunch of times when I thought there was no chance).
Part of it is that I think the opposition aren't being forced to respect outside shots, so they collapse and prevent passes into the slot or though the middle for a one-timer. Now, we don't have the shooters to change this, although Hamilton might make a difference, or Gio has to take a shot from a pass on that side from Gaudreau. But there can't be two passes around the perimeter before the shot.
Or they admit that their skill set is not for that kind of play and just work it to below the goal line and bump it into the crease, and then hope for a dirty goal. They surely have players who can score in tight.

Or go radical... this was sort of suggested on the radio.
Initial faceoff - send out Hathaway or Lomberg lined up against their best forward. Slash their stick - make sure it breaks. Viola - a two minute 4 on 4.

How long until opposing teams start taking penalties instead of using a timeout? Nothing shuts down the Flames momentum quicker.

I really hope we are able to sign Adam Fox. He looks like he could be that PP quarterback we need.

It strikes me as fairly naive to imagine that the professionally employed coaching staff has not seen what a bunch of armchair critics from the comforts of their living room are seeing. If they are not implementing changes that you imagine they ought to be there is likely a good reason.

I see a lot of hesitation with the puck and lack of movement which I attribute to nerves and frustration on the part of the players. Beyond that, I have no idea. But I do believe there is a lot more going on during games, in practices and behind the scenes that we never see, and which contributes both positively and negatively to players' on-ice performance.
