I don't see where that is tough luck. Johnny clearly threw the stick where he shouldn't have whether there was intent or not.

I've seen it called but only when the player obviously tossed the stick intentionally to break up a play.

Out of all the places his stick could land it lands on the puck after a dominant shift. I think that's bad luck but Johnny should know better

Usually only happens on breakaways, in which case it's a penalty shot.

Its rare but it does happen. I've seen it called against goalies a few times. I think Hasek might have a few of those.
