Anything claustrophobic, I remember watching The Descent and feeling some points of terrror when the girls are stuck in an underground chamber. Buried with Ryan Reynolds also evoked that same kind of terror for me.

I used to get freaked out at the simplest campy predictable horror movies. It didn't matter what kind of horror movie. Even a comedy horror. Now I'm fine... I think. I haven't watched The Ring yet though.

Sharks.
Don't know why. I've never even seen one. It may be because I watched the movie Jaws when I was young. Even as a teenager swimming in an indoor pool frightened me because of the possibility of what was lurking below. Even lakes make me nervous.
Now I live in Perth, Australia where there have been 4 Great White fatalities in the last year. I swim in the ocean but I panic almost the entire time. What the fata was I thinking?

I'm a claustrophobic coulrophobe. Absolutely terrified of enclosed spaces and ANYTHING related to clowns, those things are Evil personified.
Pylon, even your suggestion of driving by a clown standing on the side of the road on a dark night makes me feel nauseous.
As for the claustrophobia, I can't even watch that Ryan Reynolds movie "Buried". Might have been the best movie of the year, but I'll never be able to watch it.

Bees/hornets/wasps.
When I was young, and at a summer camp, I was swarmed while kicking a haystack to retrieve a lost arrow that missed the target. I was stung over a 20 times on one single leg and ever since, I turn into a pansy when I see anything with a stinger flying.
I've forced myself to become much stronger since becoming a father, as I don't want to pass the fear onto my son. No matter how well I put up the casual front when near one though, deep down, they freak me out.

Anything that could be catastrophic and world ending; rogue asteroids, blackholes, loss of the earth's magnetic field, end time solar flares, etc. I really need to educate myself more as to the probabilities so I don't worry myself. I guess I really just don't want to die in a panic, I hate epic panic and anxiety feelings.
