When this happened during my renos we just hit the nail back and forth until it was loose enough to come out. There were a couple we had no luck but we just cut them off, put the flooring on top and called it a day.

A crowbar with the little notch for pulling nails should work, assuming there is a gap beneath the head of the nail.

I agree it would work but then you waste a perfectly good excuse to buy an angle grinder

It'll probably be more effort to pull out than to cut off the head, dremel or grind flat, and cover up.

A nail that went into concrete via a hilti is probably not going to come out with a nail puller... If it does, you're a bit lucky. Be prepared to grind it off.

Simple solution ... presuming you can get a claw under the head.
A good hammer with a long handle and flat claw like an estwing. A rock or any hard object wedged in as far as it will go towards the end of the claw for extra leverage and out it comes.

You may be able to pull it out with a long crowbar, but I'd grind it off. It took a tremendous amount of force to insert it into the concrete. It'll take the same amount of force to remove it.

When doing my basement I ended up having to move a wall. I used one of these to get out the nails that had been hilti'd in.
Now I did that because I needed to move the entire 2x4. If you just need the head removed, I would also use the angle grinder.

If you just need the head removed you don't need an angle grinder, you just need to tap it back and forth with a hammer until it breaks. Takes less than a minute.

A nail string enough to be shot by a gun into concrete isn't to simply break that easily. Certainly not like a regular nail would.

A concrete nail is hardened steel, it will come out with a crowbar but it could pull some cement out and create a small divot when you pull it out. If using a grinder wear your ppe cheap grinder wheels are known to break apart with hardened steel.
