Him and Amorak couldn't stop bitch slapping each other, so eventually both of them were banned. Amorak came back as THE SCUD, and Algernon is happily taking a CP hiatus.

We do try to catch returning posters, and of course it helps when we get posts reported to us when they recognize that somebody is back.
There are a few posters who we have polite exchanges with over email; and the decision is made to allow them back under a new name. And we are often forgiving as long as the poster promises to flip their behavoir.

Well, sometimes people come back without us knowing.
So if someone can come back without us ever figuring out who they are then by definition they're not doing whatever it was they were doing that earned them the ban in the first place, which in the end is the actual goal of moderation; to modify behaviour.
In some cases people came back and it wasn't until they'd been back for a long time that someone figured it out. And by that time the person had demonstrated modified behaviour.. So it becomes a tough call for the mods. On one hand I don't like appearing soft, but on the other hand some posters are good except for (insert feature here) and if they can get a handle on (feature) then sometimes it makes sense to just let it slide.
Or in some cases people come back and just earn themselves enough infractions to earn another ban.
I've raised this idea too with the mods before and it's something that would probably be included if we ever re-wrote the rules and made the suspension/ban system more robust: Eventually a poster earns enough infractions to warrant closer attention, or has demonstrated no desire/ability to change despite repeated requests to do so. In those cases we'd have a graduated system of suspensions, which either get progressively longer (so they don't get a lifetime ban, just longer vacations), or are relatively short but are on low tolerance (someone who couldn't stop circumventing the profanity filter would eventually earn 2 weeks off each time they did it).
Maybe a new rule and consequences system is something that I should open a thread on to do collaboratively.

I really miss Reggie, he was a standup guy.
there was this one time on a Saturday when we were in the tree house reading comics and our two buddies Teddy and Vern came running in and told us that there was a dead body on the other side of town.
I of course wanted to go, but Reggie decided that he'd rather read comic books.
Teddy ended up becoming the most un popular character in the star trek univers, and Vern, well he ended up being in the worst 3d movie ever made and got his junk chewed off.
True story.
things might have turned out different if we'd only gone to poke a dead body with a stick.
