Since I have worked most of my life in a place where the bathroom would make any sane person fear for their lives, my favorite perk at work is a clean toilet, with soap, hot water and paper towels to dry my hands off on.

Couple odd ones, free car wash, dry cleaning and company shuttle. More useful though common - company stock, 3 weeks vaca + lots of possible unpaid holidays,
We also have a university program on campus which used to have classes taught by profs in the SF Bay area, so you wouldn't have to drive to university. I'm told that used to be a lot more active. There's also an outdoor theator at work, I haven't seen it been used yet so I guess that was something more for back in the hayday.
Not my current employer, but when I worked at Cisco we got one weird perk was that Cisco had a deal with some biomedical company where we could get tested to see our genetic susceptibility to 22 common genetic diseases. Ultimately I didn't make use of it though.

Appreciation BBQ every quarter. Last time we had a casino night along with the BBQ, as well as a pig roast. Every once in a while we'll get a movie afternoon for meeting deadlines, targets, etc.This week our department is going out for a curling bonspiel. Beats the heck out of the last place I was at. Their idea of rewarding work was more work and less pay. And the once a year pizza from BP's.

I get to purchase items from work at a ridiculously good price. Borrow pretty much anything I need too. Work at a pawnshop so there has been alot of nice perks.

Free internet, free phone, and free drinks including beer. However, since we're a consulting shop, I'm rarely in the office so no free drinks, unless the client has them. Which is the case currently.

I've had some pretty cool ones in the past and some in the present.
Plenty of office lunches and BBQ's. Access to pre sale tickets for huge concerts and events. Sometimes, free tickets to concerts and events. I've been to numerous movie screenings. I've been to plenty of Lions and Canucks games and have a media pass for both to sit in the press box. And I am lucky to have a job where I can put the Flames game on the TV while I work.
