Sometimes, I have to plug in the headphone jack and pull it out a few times at the back of my Mac before the sound works again. Seems like something gets stuck in there and thinks there are headphones plugged in when there isn't.

I have read, but never experienced myself, that what is happening in these cases is that the little door in there that opens to reveal the optical connector gets stuck open, and the Mac thinks it should be doing optical line out. Apparently, if you peer in there, you can actually see a red light from the optical system.
Removing and reinserting a normal jack eventually causes the optical door to shut and normal operation to resume.
