I really hate when there are two doors, and someone coming the opposite direction insists on going through the already open one on their left (my right) instead of opening the one on their right. Who walks on the left? Seriously.
I've begun to just stand in the doorway and stare blankly at them until they figure out they can go through the door on the right hand side.

I had the oddest door opening the other day. I was about to go through the entrance door on the right (had to push it) and this older guy (probably only 55 or so, fully able bodied), waits for me to open it and walks through while my hand is outstretched and I haven't gone through yet. Meanwhile in the mall all the other 9 doors could have easily been opened, but he didn't want to take his hands out of his pocket to open the door himself. Weird. Was kinda pissed as he had no excuse, save laziness, and didn't say anything as I awkwardly held the door for him (those damn heavy mall doors), but after a few seconds thought to myself how odd that was.

What about when it's a solid door (i.e. not see-through) and you go to open it at the same time someone pushes it? Who has the right of way? This happened to me today at Sears - I went to open the door and from the other side it got pushed open at just the wrong time. My response was to take 1 step back, laugh a bit and say "Oh, oops, sorry" as this old cronie almost smashed the door into me and my baby. She just gave me a rude glare, said absolutely nothing, and walked through the door first.

The trick is to time the arrival so that when you get to the door you either don't have to hold it open for anyone or you hold it open because the other person is very close. Never arrive in that 'grey zone'.

Today the girl ahead of me at the store pushed the door open so hard that she absolutely crushed it against the metal framing, and since it was a two door entry I commended her on her ability to open a door normally on her second try.
I see door etiquette as I see construction etiquette, he/she with the heaviest load moves first (does not apply to painters or dry wallers, sorry). On the job if I have beams on my shoulder and we meet in an entry, I get the first move. In real life if you have a baby/are elderly/have a wheelchair you get the first move. In that order.
Otherwise I agree with some of the other postings, if they aren't going to make it through the doors before they close; a) but are close enough - you give it the extra shove on your way through, b) aren't close enough - let them rot.*
*unless they are cute.

I generally say it twice when people do it for me, but no, I wouldn't say it's required. Just the first time as an acknowledgement is good enough for me. I mean, for the people who don't acknowledge it, how hard is it to say a simple thanks, or just smile and nod if you don't speak English?

So when you guys say holding the door, you mean walking through and holding it open a bit for the next guy (because that at best deserves a barely audible muttered thanks, unless it is some PITA door that requires using a card or entering a code), or opening it and letting the other person in ahead of you (That is definitely deserving of a well articulated thank you).

This is where you should be smart.If there are 2 doors and you get there first. Pull/push it open and hold it. If a pull door, let the other person walk through then follow them in. If push door, open it, walk through, step to the side and hold the door. If that person has any courtesy, they will thank you and hold the second door open for you. In which case, you should thank them. If you're the first person to walk through, hold the second door open.
