I finally got mine today. It says the following in the letter included with the new adapter:
If you do not arrange collection of the return envelope/box within 10 business days of the date on which you receive this letter, Apple will have the right to charge your credit card a non-return fee in the amount described at the time of order confirmation, as you agreed when you requested a repair from AppleCare Service.
I never game them my cc number, although it seems to be a form letter than goes in every box. You have to take your old adapter and drop it off at a UPS store.

I'm keeping my original power adapter. It I would be able to take my current one to the store, and immediately trade it in, I would do it. But I'm not going without the adapter for any length of time.

The way it usually works is they send you a replacement, then you send your original back to them.

Who is in charge of this product and why haven't they been fired yet?
Oh ok, I didn't know that. A friend of mine bought an iPhone the same time that I did, and they promptly took his adapter away from him, and said they would ship him a new one.
