Pretty bad. It's the exchange rate plus 2-3%, plus up to $5 per transaction, depending on your bank account type.
Personally, when I go to the States, I bring travelers' cheques. If you're an AMA member, you pay no fees to get them, and they're pretty easy to cash in any touristy place. Any hotel or any of the Guest Services places at Disney World will cash them for you, as well, every store on Disney property accepts them same-as-cash (the only places you might have a problem are the smaller vendor carts because they don't have as much cash on hand). Supermarkets and places like Target or Wal-Mart accept them too.
Most things, like toiletries and clothing can always be purchased when you get there, if you forgot something. You might have to pay a horrendous markup if you're on the ship, but if you can wait until you get to Orlando, there are a Wal-Mart and Target not too far from the Disney property (the Wal-Mart is open 24 hours, and I think the Target is too).
The things you really don't want to leave behind are the big-ticket items that you don't want to have to (or can't) replace when you're down there...camera, iPod, contacts/glasses, and your cell phone if you want to pay insane roaming charges to stay in touch. Most hotels have free in-room ethernet, so a laptop is also a good way to keep in touch if you have one.
