Usually an assistant coach fills it out and the head coach reviews it.

Based on the response by the Sharks though I think they knew what they were doing and just hoped it wouldn't get noticed.

It was on purpose. Injury happened after the lineups were submitted and they took a chance that nobody would notice and even if they did, nothing happens so worth the risk.

I realize this is an NHL thing, but accident or not, how is playing with an ineligible player not a penalty on top of not getting to use the player.

No incentive for the Sharks not to try and pull a fast one if the only consequence is that at some point someone might notice and they lose a player they wouldn't have had anyway.

Hrudey or Ball said it would have been a penalty if the Flames had pointed it out before/at the first stoppage of play.

But I agree that it should be a penalty regardless.

And what if said player scores a goal? Does it get disallowed?

Maybe the guy was such a plug that they were willing to take that risk...

Probably to prevent a team from waiting till its beneficial for them.

IE. Already on the PP, late in the game, etc.

What a stupid rule - what if they don't know until the second shift because the guy hasn't gone on the ice yet?

"Sure, they cheated and it should be a penalty, but you didn't notice in time so too bad!"

Not to mention, why does the Flames noticing even matter. What other penalty or infraction is reliant on the opposition noticing? It's only offside if the Flames notice and someone raises their arm like a soccer player, it's only high sticking if the Flames coaches yell for it before the play stops.
