Checking for dietary deficiencies is not routine and is often not what one would think would occur. For instance, iron deficiency is often simply inferred from an abnormal CBC or a low hematocrit rather than any direct measurement of iron. The problem is that other conditions will produce an identical picture.
In the case of B vitamins, most physicians will assume a patient is not deficient unless the patient is a vegetarian.
