I think it's a little of what you say and also a bit of what Pepsi said above, that people are thinking that this extreme scenario could never happen. I think it's also a little special with Trump too in that the guy talks a lot of ####, says things will happen when history has shown that they don't, so it's reasonable in a lot of ways to take his bluster for bs.
A lot would need to happen for the US to annex Canada. Trump would need to get total and complete buy-in from every gate keeper in their government for it to get to that stage, not to mention buy-in from the leaders of their military. The international community would need to be completely silent too while this is all happening.
The one way I could see this maybe happening is with a US-style color revolution setup by the CIA in Canada using the same playbook they've used in many other countries. Foment discontent of the Canadian government and leadership within a good portion of the citizens, plant puppets within that segment to promote American "democracy" and instigate protests and threats of revolutions and create a situation that is chaotic enough and destablizing enough where the "beacon of freedom and democracy" will need to enter the conflict and intervene, eventually ending with a US hand-picked leader to take over to put in place US-friendly and US-centric policies.
