What if that license wasn't tracked down? Think of the inconvenience to the person of having to get another one and not being able to drive till they get it.

Nobody has said that it should happen, or that it's a small mistake, but things like this DO HAPPEN all the time. It's the equivalent to getting change for a ten instead of a twenty, it comes down to a clerk who doesn't care about their job and isn't paying attention to it even a little bit, or possibly someone who's overworked and simply gets things mixed up.
It shouldn't happen, but it does. I guarantee you that you've made mistakes at your job just like the rest of us.
