It has happened but only when the whole command crew has been killed.

There was a court marshall of a young medical officer(who had a rank equivalent of 2nd lieutenant) who during a ship battle in the 17 or 18 hundreds took his wounded captain down to give him medical aid during a ship to ship battle. The rest of the officers were killed and command fell to him, and he was actually charged and tried because he was unknowingly in command of the ship when it was surrendered.

But I thought it was a stupid piece of writing to put him in Command, especially on a major combat vessel like the Enterprise. (From what I understand, Ensign isn't even really a command rank. Its equivalent to a trainee at Macdonalds)

An ensign could end up in Command of a shuttle if the officer in charge and only other member of the crew was killed, but nothing like a ship where pretty much every officer outranks him.

It was weak they could have promoted him in the next movie.
