If I understand correctly, players can be medically cleared while they are still injured, as long as they are physically able to play and playing won't make the injury worse.
Part of the trouble is that hockey players don't like admitting they're in pain, and doctors have to rely on their patients to tell them how much pain they're in. Too often, the result reminds me of Bobcat Goldthwait falling down stairs (in one of the Police Academy movies, I think):
‘That didn't hurt! That didn't hurt! That didn't hurt! … That hurt.’
