I still don’t get going for it at 4th and 1 from Indy 28th with 3 minutes left in the 4th quarter.

Well I guess credit to Hackett playing to win, although uncredit for a dreadful playcall. Maybe he knew he was fired either way and went out with a bang. Its pretty unthinkable him being fired is even a serious possibility after only five games, but he truly does look overmatched.

He's on the take.
Going for 4th in 1 in that situation is a bit higher risk than most nfl coaches play it. Instead he decided to play for a 4th and 6 or go home and set it up so everyone knew it.

Or at least line up in some kind of run formation, or RPO or something? They straight up set up for a 4th and goal from the 5.
The only plausible explanation is they didn't know it was 4th and 1. The coaches and refs seemed confused about downs and yards to gain for most of the last 10 minutes.
