So the Dr. says surgery could be possible... but it would be up to the surgeon. So he is referring me to Dr. Ian Lo. Looks like this guy really knows his shoulders from researching him.
Sports med guy said I have to be very preemptive on pre surgery prep work in case surgery is a go. I've been lazy this past month so I have lost muscle mass. He also said something about a wingy-something scapula (can't remember what it was called) That was something new.
What I really liked about this guy was he said "I would never tell someone not to do what they love. You do it until you can't" and I'm not there yet

Dr. Lo is the second big name for shoulder surgeons in Calgary. Him and Dr. Boorman (who nuclearfart mentioned) are the two most experienced in the city.
It sounds like the second opinion from sports med went well. Given that the consult with Dr. Lo will likely take months, I would think about that second opinion from another physio. Especially if you've seen yours for this long and they have never mentioned scapular winging given the importance of the shoulder blade for shoulder stability.

On the topic of shoulder advice - how long before you should start to be concerned about a shoulder injury? I strained my rotator cuff a few months ago, didn't think too much of it. Just hurts a bit when I have my arm above my head. I figured if I took it easy it would get better over time, but it hasn't yet. Should I be seeing a doctor (or going for physio)?

Your doctor won't treat your shoulder, only prescribe medication for symptoms, send you for an ultrasound (many physios can do that anyway), or erroneously suggest cortisone.
The sooner the better for physio. The longer you've had an issue, (generally) the longer it takes to resolve.

Update: I still haven't heard from Dr. Lo... is more than 4 months to see the surgeon for consult normal?
The past week has been pretty rough for me. Went to the chiro and he realigned/popped back in my shoulder last Wednesday. Usually the day after I'm pretty sore, but this time it stayed sore all weekend. So I went back on Tuesday and "pop" back in again. Last night while I was sleeping it "popped" and woke me up. 2 days later I'm still in pain. I'm so frustrated!
Someone tell me there's light at the end of the tunnel? And usually how long does it take to get the call for an appointment?

In my work, I pay Dr. Lo for consultations privately, and there's still a wait. If you're waiting for a consultation through AHS, I don't think that 4 months is that too long for a specialist referral.
If I might ask, what physiotherapist are you using? I recently had lunch with a physiotherapist we often work with, and she didn't mince words about some physiotherapists around the city that aren't properly trained or otherwise don't have the requisite expertise. I can give you her name if you'd like to try a new clinic.

I was refered to a Plastic Surgeon for hand sugery in April. Saw the Surgeon in mid December and had surgery in late January.

Well, I don’t know about Puxlut but I’m keen to hear recommendations. I’ve been having back problems for years. The last physio I went to saw me twice and basically said “I dunno. Maybe try Chiro?”. Disappointing to say the least.

I think any kind of stretching that simulates rowing is good. Then, do some rowing.

I wouldn't be surprised if it took 1-1.5 years to see Dr. Lo.
I'm not going to beat a dead horse or continue to try to give advice when it's not wanted but I'd highly suggest trying Evidence Sport and Spine North for physio.
As Dion and Peanut mentioned, not all therapy is created equal and you need a lot of it now and a lot of it if you have a surgery. You're waiting anyway so now's the time.
I also extend that recommendation to anyone else here. Evidence North is overall the best clinic in the city for physio. They also have a radiology clinic in house and some physiatrists.

I'm willing to do anything I almost dropped a case of pop an hour ago and went to grab it with my bad arm and now one side of my hand is tingly. I'm icing it now. This week is just getting worse.
I live down in Bonavista, so would prefer not to drive all over the place... is the South clinic just as good?

I like my physio, so I won't name him
I was talking with someone that paid out of pocket to do his surgery (similar to mine) and said that was the best thing he has done.

I waited about 9 months to see Dr. Lo. I dislocated and subluxed multiple times at the SC joint (collarbone). He told me it was an extremely uncommon injury, especially in a young female as I am and then promptly told me he wouldn't touch me with a 10 foot pole to do surgery on. That opinion was backed up by my knee surgeon.
Honestly, my light at the end of the tunnel was finding my athletic therapist. After 4, yes 4 MPFL surgeries and this SC joint problem, I truly don't know what I'd do without him.

I can't speak for the therapists at Evidence South. May be hit or miss.
Unfortunately I've found many physio clinics to be lacking in South Calgary. Conveyer belts. I'll try to dig up a recommendation around there but the drive to Evidence North would be well worth it.
Again I can't speak for your case despite seeing your MRI because I haven't assessed your shoulder (the two can be extraordinarily different) but you may want to save your money for the rehab portion. There's a chance that Dr. Lo won't recommend surgery whereas if you go privately they'll take you as a patient regardless of outcomes. And again, this time beforehand is an opportunity to get your shoulder better in case of a surgery. Surgery will not fix you. In fact, you will be way worse afterwards. But if there is a structural problem as an underlying cause then a repair can push your ceiling of recovery higher.

After the referral to a pilot project at the end of July, I saw Dr. Lo with in a month and got my date for surgery today... December 19! He said He's doing a capsular plication and likely a repair to a tear (likely that GAGL lesion that was mentioned in my MRI) He said 6 weeks in a sling and likely a year before I can get back to work 100%. If it means I can swim again after that, I'm STOKED! He said it should be successful, as he's done similar operations on swimmers.
Christmas is going to suck this year, but will be totally worth it!

Good luck to you! Everybody is different with healing times. My cousin was told the same thing (6 weeks sling, 1 year before returning to play hockey). He was back on the ice by 6 months playing. So you could be back sooner than you expect, which would be awesome.
