I can adjust the ringtone volume no problem, and when playing music or a video I can use the in-app volume controls, but I cannot figure out how to do it for a phone call.

1. Make a call and once connected, go to 'Settings ->Sounds'.
2. Slide the Ringer and Alerts volume slider back and forth and then set it to maximum (far right).
3. Return to your call, your volume should be set to maximum.
Worked for me on my old 3gs running iOS 5.1
Edit: That task switcher method sounds the most elegant. I'll have to remember that.
