You are probably SOL if you try to go through the MyCRA route or the previous accountant.
BUT if it's something you really need to get your hands on, if you retained all the information used to prepare that tax filing (which is supposed to have been returned), have another accountant plug that information into a tax program and make sure the output matches the information on the NOA boxes? You're essentially re-creating the return. But something like this might be more expensive than a regular filing because you have to ensure the recreated T1 was filled in exactly as the original T1 was filled in.

People move and/or put things in boxes and can't find it when someone like the bank needs this information yesterday. Garden variety forgetfulness, dementia and even sudden deaths and family have no idea where the info is is the more common one I've fielded. Basements flood, fires for destruction etc. I've heard as well.
I've heard different stories.
Older people aren't great at keeping digital copies either, but you don't really want to keep digital copies of this stuff without some serious data protection and encryption. It's pure identity theft buffet material.
