I think the problem with the instigator penalty is that it always comes with the 10 minute misconduct, which probably makes some refs think its too harsh to actually call, even if its deserved.
Maybe 5 + 2 would be better?
Adding a 10 minute misconduct could be reserved for very extreme circumstances.

I'd be fine with just an extra 2 but then think it'd get called too often. I don't like the instigator anyway
