I'm in the same boat as you (age-wise), opposite problem though. I feel like I've spent too much time being a youth, and pissed away my future. I'm getting to the point where I should have a career in mind, but I'm still just winging it.
I'm afraid I'll live my whole life without ever finding a career, and just bounce around from one job to the next, never really establishing myself.
EDIT: or is this what you meant? I may have misunderstood

Death itself isn't scary; it's the knowledge of your own doom and nothing you can do to put yourself out of misery
- falsely accused of a major crime and end up in a max security prison; (in a foreign country for more special effects)
- wake up alive in a coffin and have enough oxygen flowing thru to keep you alive
- get trapped in a cave and no one knew you went there (ie: the movie, "descent")
- stranded somewhere where you will surely die from hunger or animal attack; stranded in the ocean is bad but it won't take long till you drown.
- getting my leg trapped under a rock or a tree and I have to make a decision to free myself by cutting my leg loose using a small pocket knife 'cus I will certainly die otherwise.... and die anyways from blood loss.

Something happening to my family is #1.
For myself:
- Spiders, although this is getting better. I can kill them now without too much hyperventilation.
- Choking, and being alone when it happens.
- Going blind.
- Being in an accident and left with horrible brain damage.

Literally dying of thirst is pretty high on my list. I'm also pretty fearful of paralysis and blindness as others have already mentioned. I remember reading once that being deaf is actually more alienating than being blind...but I have to call b.s. on that one...I think I would actually be alright with deafness. Oh, and being in a concentration camp would be horrifying as well.

Flying or a plane crash doesn't phase me much. I've always loved flying. I'm not afraid of an accident as I have been in a serious car accident and know how I handle the situation.
Lots of people said public speaking, if I could public speak as a career I'd die a very happy man (I'm currently working on it actually).
My biggest fears are cold calling, or making small talk at an event where I don't know anyone. I get so anxious I almost get sick. Hate it, Hate it, Hate it, Hate it. But working on it. Oddly enough if you come up to me and start talking, again no problem.
So for the advantage of people like me skimming and not wanting to read the posts:
1 - Cold Calling
2 - Starting Small Talk/Mingling with Strangers

I know this is strange but large objets in water, the submerged part of a large boat for example something about it just chills me, as well as shipwrecks.
Also anything happening to one of my siblings, and bats.

While that was scary enough, I'm more fearful of what my brain is going to conjure up tonight when I sleep. Stupid wanker thinks he can always get the best of me.

Actually becoming a father is probably one of my biggest fears. I only have one male cousin with my last name. If he doesn't have kids my dad and my uncle's last name will die (at least from our immediate family).
The closest I'll come to having kids is a cat.
