Liver (except in pate, that's delish)
Brussel Sprouts (haven't had these since I was a kid - blegh!)
Egg Salad and Potato Salad I'll pass on.
I also really don't like mayonaise, and I don't know why since everyone else seems to like it. I'll probably eat something with mayo on it, but I would never put it on otherwise delicious food, myself.
I love a lot of the stuff in this thread though - fish, asparagus, squash, sushi, olives, pickles, mushrooms, tomatoes, etc. Just writing them down is making me hungry. Different strokes.
As far as eating insects goes, I once ate about 10 moths as part of a competition. They certainly weren't good, but they weren't as terrible as I'd expected. I'd never eat a cockroach though.
Anything alive, at the time. So I'm talking insects, bugs, whatever.

I was raised on marmite soldiers - delicious! I still eat marmite nearly every day - not as much as when i was still in England as marmite is SO expensive here - on hot, buttered toast. I do, however, hate Vegemite.
I do not wish to eat human foetus, nor do I particularly enjoy sucking marrow out of a bone, sucking brains out of a skull or eating colon. I am pretty willing to give a lot of things a shot, but I would definitely draw the line at human foetus.

I am the least finicky eater I know, so there are few things that I absolutely could not stomach if I put my mind to it but based on principle alone I will/would not eat veal or dog.

Where I live now there are tonnes of places advertising dog meat. I haven't eaten any yet, and I have minor reservations, but I expect to give it a try sometime this winter.

Oh I won't eat anything that will kill me if I eat it.
Don't know if the black market for it exists in Canada. I'm hoping not. It exists in Hong Kong though, and also in Chinese mainland areas around Guangzhou. Aborted babies are money makers, and tender eating apparently.
EDIT: I am aware that this some will say this is an urban myth, and it's certainly not common. On the other hand, I have been told about first-hand experience of a couple of friends, have read enough Chinese stories about it and am familiar enough with other strange occurrences in China to find this totally believable though I have never been in a position to order it.
EDIT II: Just to add, I don't really see anything morally wrong about eating babies unless the babies are being killed in order to be eaten. As long as they are just basically natural waste in the form of human flesh, I don't see anything so bad about eating them from a moral standpoint. Just the taboo makes it a bit unsettling.

At the moment I can't think of anything that I would flat out refuse to eat. There are some items that I am less inclined to consume, however if it comes down to it I will eat them.
To me people who have a certain main stream item that they refuse to eat are being immature and somewhat childish. On some level you should be able to at least try a piece of whatever is being served (allergies pending) even if it's not something you favor. For example I have a friend who is very picky about what he eats, the last time I had him for dinner at my parents house he wouldn't eat half of what my step-mom had prepared. Ever since than he's not once been invited back to my house for anything, it's a courtesy to at least attempt to eat what your host has offered, even if you would never even think of making it for yourself.
