So first game i played with my soccer team in Calgary a couple years ago was also the first game ever for the team. I hadn't played soccer in 2 years making me the guy with the smallest layoff in that term. 2 of my defensemen had never played soccer and one was my younger brother who was 16 in a mens league. With all this in mind, i expected us to be bad.
So game starts and the only time in the first half that the ball leaves our end is when i punt it past half. Rolling into the second half, we finally get a shot on their net, one of our forwards cleared the throw i sent to him and their goalie picked up the ball. It was all he did all day.
About halfway through the second have, we are only down 8-0 at the time, im getting tired and sore from seeing so many shots. The other team gets a shot from the top of the 18 yard box coming down the left side, i make the diving save, sending the rebound out to the right, straight on the foot of one of the defensmen that had never played before. There is no one near him at all, so after diving i get up slow thinking he is going to clear it. Instead, he winds up and fires it top shelf over me on the ground. First goal in team history and arguably the first legitimate shot.
Another time, i was playing some friendly road hockey after school a few years back. I'm in net at the time and we are shooting around between games. My buddy takes a shot and misses horribly sending the ball about 150 meters down the street. He takes off after it and the rest of grab a different ball and keep shooting around. so i make a half assed effort at a save going down into a Bill Ranford style kick save position, not really caring as it was warm up when i look back to see where my buddy went. I look back just in time to see he had shot the ball from 150 meters behind the net, bounced it off the curb to the right of the net, which proceeded to go in off my foot. I still cant live that down.

I was center in flag football, not beer league but Jr. Highish. We get in the huddle, standard 22 on 2 play. Walk up to the line, QB calls "DOWN, SET....HUT ONE, HUT TWO," and in an inexplicable moment of brain fart I didnt move a muscle, everyone goes flying past me. Ref blows the whistle, a bit of confusion ensues. Offside of course.
The coach comes over in the next huddle, pulls me aside and says something to me I will never forget, "Kris, most people think the most important player in the game is the quarterback. Well, they're wrong, it's the center. Because if the QB doesnt get the football HE CANT DO ANYTHING!"
That was tough to live down.

Men's league final a couple years ago. Only my second year on the team, but the first time this team had been to the finals in about 15 years.
I'm the goalie, I head out beside the faceoff circle to intercept a puck heading into the corner early in the second period. First man in was an opposing player so I attempted to shoot it behind our net and rim the boards to my D on the far end, a play I had done three or four times that game already.
So I fire it.... right into my net.
I was so sick about it, I didn't let anything past me the rest of regulation and we managed to tie it, but lost in a shootout.

Golf....
Playing in the company golf tournament. I'm on the 11 hole and my tee shot lands in the woods beside the fairway. Thinking i can avoid the tree and make the green i pull out the five iron. I hit the tree and the ball ricochets into my groin area. I'm on the ground in severe pain and my forusome is killing themselves laughing.

Mine occurred while playing ice hockey. I was going off for a line change and there was a guy ahead of me stepping into the bench, so I came up just to the right of the bench door. I stopped, and not exactly sure how it it happened but I lost my balance and went face first into the boards. I managed to get my hand up and over the board so I wouldn't actually hit my face. My glove flew off and hit the guy ahead of me in the face, and my stick hit the boards, knocked me on the top of my head (thank goodness for helmets) and wound up near center ice. Thankfully it was the last game of the year and nobody remembered the next year, but I was razzed pretty good for the rest of the night.
I once saw another guy gingerly skating back into the play and trip over the center ice line. He valiantly tried to regain balance but somehow wound up leaping head first into the boards. He knocked himself silly and wound up with a concussion.

I was at a volleyball camp one summer in high school, and we were doing a drill where one person would stand in the middle while everyone else made a circle around them, and they would each have a volleyball. The object of the drill was to get a "touch" on each ball as it was spiked at you. Anyways, I'm in the middle and doing pretty good, almost finished when some arse spikes the ball right at my face. And because it was a supposed "rapid fire", when the person before you spiked the ball in the circle, you threw your ball up so that you'd hit it immediately after the other person. So, I get smoked in the face, glasses fly off after cutting my nose, hands rise to my face...and the next person promptly spikes it right at my "boys". So now I'm on the ground, one hand assessing the damage to my face, the other clutching my abdomen as a gym full of high school students laugh at me. To this day, some of the people that went to that camp greet me with "Hey, remember that time you got smoked in the balls at volleyball?".
