Some reports of Un being in a vegetative state and others saying he's dead. Clutching heart and dropping dead a while ago too. Hoping it's true although that would have been a much too peaceful end.
I don't think its going to matter all that much if he drops dead, there's no liberalization of North Korea coming as long as that family has control, and the Military is perfectly content for that to continue to happen.
His Sisters, are both equally ruthless and nasty as he is, the only difference is that they will have a softer international appearance.
The problem is that if any of the other factions or Chinese decide to get involved to depose the Kim family, frankly it will cause a major civil war with the possibility of nukes or chemical weapons being used, and a massive refugee humanitarian crisis.

With how secretive the Kim family is it is impossible to say how this will play out. Any random person could be brought out and claim to be a Kim while another power pulls the puppet strings.
It is still unknown how many kids Kim Jong Il had. Kim Jong Un was only introduced to the North Korean public and majority of the government once his father decided he was the heir back in 2009.
You could have multiple supposed Kim lead factions vying for power.
There's a pretty good sense of the size and members of the Kim family due to defections and South Korea's intelligence service is very good.
They had been moving one of the sisters into the same ascension path as Kim Jung-Un a while ago, she also serves as the head of the defense committee which probably means that the army is extremely loyal to her.

With China already having dispatched a team to NK, I'm sure they are all over this. No saying yet which way this is going to go if he is actually dead.
