This story is now making rounds internationally on different companies health and safety bulletins. The version I read said that the guy was doing maintenance on a machine while it was running. His shirt became caught and he got pulled head first into spinning blades.
Apparently he was alive at the scene but died as they were removing him from the machine. He was so badly injured that they needed a forensics team to identify him. Those last several minutes of his life must have been a goulish nightmare.
