It took a near perfect throw from Loop to just barely get him.

But then why don't teams do it more often? Was there something he saw that made him think he'd have a good chance?

I can't respond to the 2nd part.
As to the first, surprise is key. And pressure is a bigger factor in bigger games. And the Jays had already dropped a ball at 3rd on a stolen base, thrown a ball away at first, and made several bad throws defensively.
Also, Loop owns Bradley Jr, apparently. So the odds were against Boston with 2 down.
