For added fun, when forming the patties myself I put sliced olives in the middle of the meat before cooking. It probably sounds gross, but I've done it with both green olives and black olives and its fantastic. (actually, credit for this invention goes to my dad)

Fry onions in a little bit of butter so they become nice and sweet.
Put block cheese on patty while it cooks. Do not use those slices.
Toast bun. Preferably on a BBQ where you are also cooking the burgers.
Crown
Onions
Cheese
Burger
Lettuce
Tomatoe
Heel
No condiments needed.

I don't worry about most of the order, but a little bit of mayo on each bun really helps to keep them from getting soggy.

Top Bun
Mayo <== Protects bun from sog, if using real mayo
Salad (Lettuce/tomato/pickles/onion)
Condiments
Patty
Real Cold* Cheese <== Protects bun from sog
Bottom Bun
*Melted "real" cheddar cheese has a weird texture and colour, only processed cheese is better melted.

That no ketchup after you're 13 rule is dumb. How can anyone not like the taste of ketchup? Oh, it's going to take away the authentic purist taste of my hamburger or hot dog? Sure of course, yes please. The taste of ground and processed salty meats gets boring after awhile if not countered by a burst of sweetness.
