From that original photo, it looks like the moviemakers are going to present the alternate version of the origin of the Joker (Tim Burton's Batman supported the comic-book story by having Jack Napier fall into a vat of chemicals). Another version had killers wiping out the pre-Joker's family and slashing the corners of his mouth in such a way that the scar gave him an ugly, permanent grin. That might explain the absence of green hair, if that is indeed the case.
