My personal favourite when delivering when we had a time guarantee; I had taken the order myself, and it was a cold night- around -30. (Winnipeg) The guy asked if I could do him a favour; he was alone with his baby and asked if I could stop at 7-11 and grab a litre of milk. I told him we couldn't do that with our time constraints, etc, and he pleaded. He explained his daughter was asleep, he didn't want to wake her to dress her up for a 5 minute drive.
I agreed, and got the milk. There was a bit of a line at 7-11, and I ended up getting to his house a few minutes late. He insisted on getting the $3 off the order. I told him I was at 7-11 within 30 minutes, it was his extra errand that made me late. We went back and forth and finally I relented. Then he asked how much he owed for the milk, and I told him he wasn't getting the milk now. He was pissed, paid the full amount for the pizza, paid me for the milk, and slammed the door on me. (no tip of course)
The best part, he must have told the story to his wife, because the next day she came into our store and gave me a $5 tip.

Sure, the tip is implied but it's not part of the price. You always check. Always. I asked as the money was being handed to me, before I counted, that removes any awkwardness of the extra 5 obviously being for a tip. It's just a courtesy...

Well yes, but my cashier doesn't usually get a tip, and my pizza guy does.  With the cashier it would clearly be a case of an accident. With the pizza guy, it's likely a tip. Never does hurt to check though, people can get bent out of shape for a lot of reasons, and money's a big one.
I used to work the phone line for a semi-national chain of pizza, and this lady calls up at 3am (when all the stores in her area have closed) and starts going in on me about how her delivery driver shortchanged her a dime. She was insisting that we wake the store manager up and send him to the store now to deal with her issue. Ended up telling her that if she still felt the same in the morning after she'd slept, to go to the store herself.
