Assuming John was your grandfather.
John & (Grandmother's Name)
Or have separate signs for men's and women's. Keep the Current name to the left and then put a female oriented name to the right. You'll sometimes see chains with multiple brand stores do this (Tan Jay 'n' Alia; Le Chateau/La Chateau).
Don't pigeon hole yourself into thinking you need one name.
But I like the idea of adding your grandmother's name as it shows female and keeps the nostalgia of ole. Could be some good talking points around town. "Oh did you know that's the name of John's wife, what a nice little touch!"

I agree that by sticking with just "John's," you're still stuck with with a male clothing store image. But coming from a family who ran a family business until everybody interested had died, I know where you're coming from with regards to the name.
John's Apparel, John's Outerwear, John's Fashionwear, etc. . . . no matter how you slice it, I don't think these types of name differentiate enough from the existing name to be worth a change.
Some options might be:
J. Spetch Apparel, John Spetch Apparel or Spetch Apparel*
John Spetch Trading Company or Spetch Trading Company
Spetch's (maybe drop the apostrophe (Tim Hortons, Loco Lous, Marks . . . I don't know why they do that.)
*There are so many words other than 'apparel' to chose from: Activewear, botique, clothiers, fashionwear, outerwear, fashion. Or nothing at all.
Ultimately you have to go in the direction you're most comfortable going.

I agree with this (and it was mentioned by a couple others as well).
John (surname here). Simple and implies a higher scale clothing store even if it isn't, to imply a "cheaper" vibe I'd include the "Apparel" after John (surname).
