IPTV multicast is absolutely more efficient than broadcast as you only send the streams that people are watching. Each stream is only sent once regardless of how many people are watching it. The worst case would be of the homes serviced by that particular server every channel you provide happens to be viewed at the same time (which is highly unlikely). And in that worst case scenario it is equivalent to if you would have broadcasted every channel.
IPTV unicast (on-demand) is what you are referring to above.
