One reason, IMO, is that they try to pass to players that are too close to them. Gulutzan talks about 5 foot passes. But the problem with that is there is no time for the recipient to react and control the puck.

They should have put this game away in the 2nd when it seemed like they had the Kings on the ropes. Instead they let them hang around too long, and here we are. Flames look like they're playing for OT now.
