I don't think I've seen that happen anytime recently.
The strap can break during a fight, but they always call it when the strap isn't tied down to start.

I think from there if that shot is deflected it goes up, not across far side. It didn't look to change direction to me.
I think they initially said it must have been deflected because otherwise it's a bad goal. Which IMO it was.
