Uhh...the twist is that 13 is not going to Rome for treatment for Huntington's and has secretly disappeared for who knows where while lying to all her friends and co-workers.

There has to be a whole season of episodes! I think it would be silly to expect the biggest twist in the first one.

The show is 7 freaking years old now. It has to do something new with the characters. If they didn't grow or change, I wouldn't want to watch anymore. It's obvious that people watch the show for the characters and the medical mystery could matter less.
Most shows are completely over by season 7.
And House already jumped the shark the 2nd time he fired everybody which was years ago.

Disappointing episode.
The season will be fine - House will still be the character we love. They pretty much acknowledged it in the episode when Cuddy was doing her 'I love you, flaws and all - you're still the greatest man I know" speech.
But I still think the plot timing was odd. House and Cuddy get together at the end of the season. Then they try to write in the "reward" scenes...4 months later. Can't really jump back into it. Very strange timing.
Edit: Really enjoyed HIMYM tonight. Sure it's a giant tease (seriously? not Ted's wedding?) but it's the interaction between the characters that keeps bringing me back for more.
