Grade I is a sprain, with minimal disruption in the ligament that is disrupted in separated shoulders (making up the acromioclavicular joint). Grade II is a bigger, but small tear, and grade III is complete disruption of the ligaments and muscle attachments in that area. There are worse injuries (grades IV through VI), but those are rare.

Usually for Grade I and II injuries, the management is conservative (sling, cold/hot compresses, physiotherapy etc.)

Grade III usually need surgery but even that is controversial.

I had a grade III separation about 11 years ago. My advice is to go see a physiotherapist and do the rehabilitation exercises they give you (i had a huge elastic band and had to tie it to a doorknob and do different stretches) and the ultrasound helps heal as well.

I went to see a sports physiotherapist a year after and he basically told me that my shoulder didn't heal properly and advised me to have surgery later, when i'm done with sports.

Although mine was not seperated I did have inflammed tendons and bone spurs out the teeth, but i was out of the sling in a couple of days and moving freely afer i think 1 month or so, but it does take a lot of physio, and you really have to work at it.

Not to sure about football, as im a rugby guy myself but the Dr said i could go back fully, 5 months post op (not going to shoulder was messed long enough)
Dr. Ian Lo is one of the best arthroscopic shoulder surgeons in North America. He is part of the Texas group and works through the sports clinic at the UofC. He has published various paper and I was considering surgery with him a couple years ago.

i seperated my shoulder a lifetime ago, i have a telltale bump and i have been told that one day i may need surgery and it will be a six month rehab period. given the wait list at the time, and the fact it only bothers me sporadically, i'll wait for it to blow up.

it was a wied injury as it hurt a lot for about 36 hours and then the pain decreased to nothing. it is surprising how many muscles in your shoulder area are involved in lying down.......
