Jump the Shark - The Drew Carey Show
Pretty much right after Christa Miller left the show. And the final season was aired in completely the wrong order, confused the living hell out of me.
Nuke The Fridge - Inglourious Basterds
When they stormed into the theater and shot Hitler up. Didn't realize Tarantino had set it in an alternate universe until that moment. Really put a dent in my respect for that movie, but not Christoph Waltz.
Unexplained?
Pretty much any series cancelled before a proper finale.

Battlestar Galactica
Jumped the Shark: Exodus, Part II (season 3, episode 3). One of the best episodes of television I've ever seen and it contains the awesome "Adama maneuver". At the time it wasn't obvious, but the show was never as good again. Maybe you could argue the show didn't really jump until the end of Season 3 (when Starbuck just magically re-appeared), but that's the one I remember most.
Nuked the Fridge: Umm... probably a couple times in season 3, and several times in season 4. Season 4 alternates between some really awesome stuff and some really whacky crap. The writers looked like they were just making stuff up at many points.

Lost.
Jumped the Shark sometime in season 2 (maybe around half-way). It became entirely evident the writers had no clue what they wanted the show to do, and they just kept layering absurdities upon absurdities, not to mention dragging the story out unnecessarily. I stopped watching at that point, but I'm sure they've nuked the fridge many times along the way (time travel!? alternate universes!?). It's too bad, because I loved a lot of the character backstories.

Jumped the Shark: Dogget and Reyes come in, Mulder gets abducted (Many think it was even as early as the season directly after the 1st movie)
Nuked the Fridge: "Bullet time" view of Krycek taking one in the head, Cigarette Smoking Man being obliterated by a missile in the Series Finale
Unexplained/What happened? You tell me? The finale was basically a "clip episode" a la Seinfeld. Apparently something will happen in 2012.
