One question though ... was he going to the psych ward to detox? Or did I miss some other massively big problem? I mean I know he was hallucinating, but wasn't that because of the drug addiction?

SPOILERS (Possibly)
When they were talking about what it could be, the last on the list was severe mental illness.
What we witnessed in the episode was beyond hallucination I think, he was literally experiencing a whole reality other than his own. I think they are hinting at something much more serious than the drugs.
Should be interesting to see what happens next season.

As mentioned, when they were going through the possibilities of what could be wrong, mental illness was last on the list. It's last because a doctor that has a mental illness can't legally practice medicine, which basically means the end of House. He thought he went through detox the night before with Cuddy, but that obviously turned out to be false as was the entire night. Essentially, House never did go through detox so that will likely be the first thing he does at the Pysch hospital, but it appears to be part mental illness (at least part, if not entirely). So this makes for an interesting season premiere.

My guess goes something like this; he goes through detox but the hallucinations gets worse, so he has to go through a self diagnosis to find the physical problem while in the grips of his hallucinations before he lapses into permanent insanity.

I'm not even sure how to process this yet. I did NOT see this coming, although I wasn't really surprised. It's like I kinda knew all along this was going to happen, but didn't realize it until it did.
Man oh man, I hate waiting for the new season to start. It's not like sports where you have the Draft and a bunch of trades and signings to discuss during the offseason. With a TV show, it's pretty much just have to "forget about it" for 4 months.

I must admit, I thought this might be the way they resolved the two big 'wow's of the last episode. In fact, I nailed it...
SPOILER: Kinda like shows or movies that do that whole 'it was all just a dream' take back. It was the only way I could see the writers resolving what happened last episode in a way that wouldn't kill the show. You just knew it had to be a dream or hallucination. Otherwise, they killed two of the biggest hooks. House's addiction, and the dynamic with Cuddy.
However it was done so well, and made so much sense, especially since it took into account problems and situations that were already going on, that I didn't feel cheated at all. It was part of the storyline and grew the storyline.
SPOILER: Instead of just being a pointless 'it was all just a dream' take back moment that can make a viewer feel cheated.
Great episode, great case, I liked how they resolved the deal with Cameron and Chase, and I love how the case they worked on lent insights into the going ons of the other problems of the characters. Kindof backwards, where usually the problems of the doctors lend insights into the case. Neat reversal.
SPOILER: Nice to see Kutner make and appearance, and it will leave some plot left for next season, without being a cheesy cliffhanger ending. Is House addicted? Is he mentally ill? Is it both? Will he ever make it back as a doctor?

yeah I just watched it and was blown away. Hugh Laurie should win the Emmy for this season of house as I thought it was one of the best seasons. The look on his face when he came into Wilson's office at the end crushed me. It was the first time you see genuine fear and worry on House's face. Looks like Schizophrenia is the most likely culprit.

I don't know about schizophrenia. I thought the whole deal was that his drug addiction had taken over, and it had gotten to the point where he was now lying to himself in order to keep the addiction going.
I think that's why they drew so much attention to him reaching into his pocket and finding the pills there.

You can have rehab at a psych hospital, but the psych help isn't as good at a rehab centre.

I agree, Huge Laurie needs to win an award for this season... excellent acting. I wonder if his personal battles with severe depression and ongoing psychotherapy are giving him the unique ability to convey those feelings through House.
It's weird and it's the way the show works. A patients sickness is actually metaphorically related to his own, but to go another step, Houses sickness is related to Hugh Lauries.

I can't figure out who the blonde on the steps is at the end. She isn't Amber, since the Doctors motion for her to go ahead of them as they go into the hospital.
Whoever she is, she was meant to stand out in that shot - which may mean that she'll have a prominent role in the next season.

I'm not so sure she stood out that much. I think it would have looked odd to just have people in white coats waiting, so they added the best thing they could... a female admin staff (ala counter-cuddy)... hmmm... maybe you're right. She walked those steps like a true hooker though
