There is a gourmet hot dog place on 4th St SW (name escapes me atm, has a French name) that has REALLY good malt milk shakes. Not quite the core but not miles from it either.
Le Chien Chaud
Doesn't Tubby Dog on 17th ave also have milkshakes. You could get the gripper and shake...mmmmm.
Of the Fast Food places, I always though A&W wasn't bad (and that is a relative judgement), but for really good milkshakes you need some sort of a Diner/Greasy Spoon.

Are they really made any different anywhere you go? The ingredients are quite simple?

Yeah, a lot of the fast food places are similar since they use similar pre-packaged stuff they dump into a mixer.
Places that make them on the fly can be plain or very good.
Hard serve/soft serve ice cream
type of milk used (whole/2%/etc)
Perhaps any sugar/sweeteners added,
but most important is what they use for flavouring (flavouring or real fruit/syrup?)
Then you start getting into the amount of each ingredient, how long to mix, etc.

I haven't tried any other flavour, but the root beer milkshakes at A&W are great. The best ones from any of the major fast-food chains for sure.
Depends on your definition of the Core, but the Galaxy Diner makes a great shake. And I saw David Moss their about a month back. If the Mossman is down with it, you can't go wrong.

I can't tell you why, but the A&W shakes are my favorite. There's some jargon on the side of the cup but I can't remember what it says. Something about the way they freeze it.

That is currently the best choice we can think of. We figure it is a 10-20 minute walk. Only open to 4pm if I am not mistaken.
Location: In a land without pants, or war, or want. But mostly we care about the pants.
Exp:
Go to hooker Mac's, buy some ice cream and some milk, put it in blender, drink.
Of course, if you're thinking of doing this at WORK, you need to change this to:
Go to hooker Mac's, buy some ice cream and some milk, break into Jammies' place, put it in his blender, drink, leave dirty dishes on counter with "ha ha!" note.
