I generally don't ban bands so much as I do songs.
"Tainted Love" in any incarnation will cause me to leap to turn the radio off. As will Smashing Pumpkins "1979", "Killing me Softly" by the Fugees and "Money". All songs that are like nails to a chalkboard. "Roxanne" is by far the worst sounding tortured-cat-wailing noise, accompanied by infants banging instruments together, to ever be put to tape in the history of the universe.
For the most part, the bands that I ban are the moldy oldies. I can't stand Led Zeppelin. ZZ Top is also impossible to listen too. X92 and Fuel have their faults, but they have almost completely replaced Cjay on my dial because I am so sick of hearing the same six songs from 30 years ago.
As far as Finger Eleven goes, "First Time" is still their best song. I like a lot of their recent stuff, but it is overplayed. Hard to fault the band for that though. Especially when CANCON necessitates it.

I had a banned list for my wedding. No Abba, no Village People, that really stupid "I Knew the Bride" song, bird dance.

I would like to personally thank Apple for an excellent substitute for repetitive radio, in any format.

Above is their best song IMO. I saw them on a side stage at an Edgefest years ago and I had never heard them before and right after the show, I bought their CD at the show itself. I had never done that before. Tip is an awesome album.
More crap that I can't stand:
Alexisonfire(the band would be great if the damn singer would stop the screaming. It's bloody incoherent)
Rihanna
Beyonce....if I had a gun, there's a good chance I would shoot her in the knee-cap, watch her bleed for a bit, then shoot her again.......yes, Im a little sick, but I cant stand that chick and her music.

Evenescence (I refused to learn how to spell their name) actually makes my stomach sick, and I want to hurt myself/others. I'm normally a pretty sane person...
Making your radio station listeners hear Linkin Park is just insulting.
I can't stand CJAY anymore, pretty must all they play is crap.
Q I can hardly stand. If I hear the Five Man Electrical Band one more time...

Most everything in the 90s, most things labeled "alternative" in the 90s. Nickelback. Linkin Park. Green Day. Aqua. I've recentely noticed turn off the radio everytime I hear R.E.M. 1992 -> 2004 is pretty much completely banned music for me. What an attrocious era of "music". It's too bad those were my junior high to university years and that's what all my friends were listening to.

I hate anything that has ever been in the top 40 since 1998.
Again I say, ANYTHING that has been in the top 40 since 1998.

Yeah that pretty much covers it for me too. Maybe not everything, but damn near everything. I think that was the year people embraced boybands again.
These days though, I hate that indie rock has become a genre. There are so many ty bands that are considered "indie rock darlings" that have no business making music, they just have the right threads and haircuts. Indie rock is as bad as corporate Nickelback rock these days.

Huh? Rise Against as corporate rock? Hardly.. I give those guys a lot of credit for the work they put into getting famous. The song Heaven Knows is one of my favorite songs, that along with Generation Lost.
But if you've only ever heard songs from Sufferer and the Witness then OK. Same kind of thing goes for Against Me! these guys have all been around for quite a few years.
