True, there are a few intersections like this in Manitoba. On busy days, it's like playing car frogger trying to get across. It is also really difficult to judge how fast people are coming with some people going 80 km/hour and others going 140 km/hour.

These intersections terrify me.
We take HWY 540 to Fernie almost every weekend in the winter and I'm still on edge every time I make the Eastbound 540 turn onto northbound 2.
And then there is the 104th St. intersection with HWY 540. When they first repaved 540 it was a 4 way stop. Now 540 gets the right of way and 104th St. is a stop sign. I live in constant fear of someone running that stop every time I go through.
I'll take an icey mountain road in the middle of a blizzard at night over a broad daylight mid summer prairie hwy crossing any day.

Have you considered taking 533 instead?

My Dad was from Carberry and my uncle, now my cousin, owns the farm a mile east of the intersection so I know it well. From the north side of highway 5 you have rumble strips, lights and a stop sign. The median between the West and East lanes is wide and the ground there is pretty flat. I don’t believe the highway has any dips on the west side of the east bound lanes. You always looked before crossing and went half way to the median before looking again.
It’s an intersection like so many others across the prairies and it would be really costly to interchange them all. From my knowledge, I have no knowledge of any accidents at that intersection, though I suspect they has been the odd one. No doubt my family heard this one assuming they were home.
Just a sad event.

Maybe dramatically increasing the width of the median and including a stop sign would be a cost-effective approach to improving the safety of these intersections.

Twenty years ago, a woman I know was involved in a fatal collision at that intersection. She was traveling south on Highway 5 as well. I’m not sure how often there are fatalities at that intersection, but I know of two.

The cost of changing all those types of intersections would be in the billions.
But I wonder what the cost would be of changing just the high traffic ones. Because there are repeated accidents at the same 2-3 intersections in Manitoba every year almost.
Same thing in Sask I'm sure.
Pretty scary and sad situation.

It would not be cost prohibitive. For example in Alberta there are 10 intersections between the two TCH highways and other major highways that are not grade separated. Maybe another 10 intersections with secondary highways. Every other intersection (gravel roads) needs to be closed, full stop. (maybe you can keep them right in, right out). There is zero need to keep access to major divided highways from every little country road, just the major highways. And those need to be grade separated. They do it with the Interstate Highway system. It's lack of political will in Canada that we do not do it here.

They offset the north and south exits which makes straight throguh a bit more of a hassle, but prevents having to cross both lanes.

You can also do a more simple bridge like this, without full cloverleafs. Hell, you could make them alternating one lane if it saved some money. These wouldn't be as expensive.
