I would imagine there are a lot more close calls then one would like to think. Scary stuff.

In the end this stuff is all made and then operated by people, so I'm not too surprised something like this happened. Though I always find these stories quite interesting.
Couple of other close calls during the Cold War. Funny you never really hear about people like this, even though they likely saved countless lives:
Well, it's only North Carolina. It's not like anyone would have noticed.
More seriously, that's quite terrifying. Three of four fail-safes failed. I really hope they did take the report's recommendation and heightened security measures from then on.
