It goes way further then that, in many European countries it is basically impossible to go to the washroom for free unless you're in your hotel or a private residence. Alot of the chain restaurants overseas (Subway, MCD's, etc.) wont let non-paying customers use the bathroom either, you have to present your till receipt at the washroom door or pay a Euro. I can't imagine any of the fancier restaurants would allow it either, though can't say I've tried. Almost every train station/metro station also charges for washroom use.

Correct. I haven't been in the night club scene in about 10 years, but when I did go I saw them at most places. Cowboys, Outlaws, Roadhouse, etc. all had them. I did my Master's a few years ago in Saskatoon and they had them in a couple of the bars as well.
The toothless 80 year old giving gummers was a story I heard that happened at Ranchmans. I never saw it personally, so it could be an urban legend.

Bingo, plus who knows if they're having problems with homeless people coming in to scrub their butts in the sink or junkies using the can to get high.
I see it all the time here especially at fast food places, I don't blame the restaurants for doing something to prevent people that aren't patrons from using the can.

The reason the bill was unreasonable in this case is that she asked permission to go and then the employee tried to charge her after she went.
Since charging for Bathrooms is not the norm the employee really needed to tell the person upfront that there is a $5 fee for bathroom use.

Lot's of places require you to get a key to use the washroom.

The washroom at the museum at Arromanches had an attendant like this. She cleaned the stall after every single person and signs everywhere forbid tipping. Can't imagine a working day this awful.

Actually, I know of at least 2 places in Dublin where you can't even do that. One in Trinity College, and one in a McDonald's. They both have ultraviolet light (or whatever the correct term is). I thought it was just some trendy thing, until someone explained to me that it prevents junkies from being able to see their veins, and therefor can't shoot up.
As for pay toilets, I don't think I've ever seen one in Dublin (apart from those space age public cubicle thingies) Did come across them at a train station in Oslo though.
